– Attend a Diabetes self management class

– Daily monitoring of Blood Glucose

– Hg A1C checked – twice a year

– Dilated eye exam annually

– Foot Exam annually

  • Diabetic Self Management Education and Support Programs can help improve outcomes.
  • Barriers to Diabetic Education include lack of insurance/ limited coverage.
  • Your physician should refer you to diabetic teaching when diagnosed.
  • It is important to know if you are prediabetic so that you can start to work lifestyle changes like healthy eating and reducing your weight.
